Texas Health Resources seeks to hire a Community Social Worker.
Monday - Friday, 8:00 am - 6:00 pm. Occasional evening
Locations: Northeast Zone: Frisco, Plano Allen & McKinney.
BSW (Bachelors in Social Work) from an accredited school of social work required.
MSW (Master’s in Social Work) preferred
·3 years of experience working in a community / outpatient setting
·Or, 3 years of experience in the geriatric field working in a direct service capacity
·Or, 3 years of social work experience in an inpatient setting
·HealthCare setting preferred, previous experience in home care setting a plus
LICENSE / CERTIFICATION:
·Must be licensed in the state of Texas
Required Upon Hire:
LBSW- Licensed Bachelor Social Worker
DL- Drivers Licensed
LMSW - Licensed Master Social Worker (Preferred)
Community Care Social Workers will provide Biopsychosocial assessments and interventions for population management of SWHR PHSC Care Coordination Risk and ACO patient panels via home, primary physician clinic, or other settings visits. Social Workers will collaborate with patient’s Medical Home team and other professionals to assess patients’ ability to comply with patient’s plan of care and/or disease management needs. In addition, Community Care Social Workers will facilitate and manage referrals accordingly to ensure continuity of services.
•Provide patient and family counseling for adjustment issues, grief, and end of life issues
•Provide support to patients and their families when personal, social or environmental difficulties exist which predispose illness or interfere with obtaining maximum benefits from medical or mental health care
•Chronic disease self-management- Instructing on coping strategies including planning, behavior modeling, problem solving techniques, and decision making related to managing chronic diseases
•Provide advanced care planning education and completion of needed documents
