Position Description :

Provides and documents individualized social work assessment, care planning, intervention and outcome evaluation for seriously ill and dying patients and their families receiving facility and home hospice and medical home care services. The Social Worker assists patients and families with issues related to self-determined life closure, safe and comfortable dying, and effective grieving, through individualized care planning, assessment, intervention and evaluation in collaboration with the Interdisciplinary Team. The social worker assists patients and families receiving medical home care services with support in obtaining needed services and resources. The social worker supports and guides the medical home care patient and family to transition to hospice services if appropriate.

Minimum Requirements :

Minimum Certifications, Registry and/or Licenses Required: PA License (LSW or LCSW) required. LSW with advanced certificate or LCSW with advanced certificate.

Master’s Degree in Social Work (1267). Two to four years of post-Masters experience, with at least one to two years of related experience.
