The successful candidate will perform psychosocial assessments and develop mutually agreed upon intervention plans with patients and families on the chemical dependency detox unit as well as in the Emergency Department. This will include intervention for substance use disorder and other co-occurring mental health disorders. Will provide referrals to community agencies that will support efforts to guide health maintenance, recovery and optimal coping. They will be responsible for implementing safe and sustainable post discharge plans. Must be able to work collaboratively on a multidisciplinary team. Experience should include behavioral health in a medical or substance abuse treatment setting. The patient population presents with significant psychosocial needs including but not limited to chemical dependency.
Full Time, Day Shift
8:30 am - 5:00 pm
Location: Chemical Dependency Unit and Emergency Department
Education: Master's degree in social work and two years of post-masters experience providing clinical social work, preferable in a health care setting
Experience: two years experience
Works in patient care areas where there are no physical discomforts due to dust, dirt, temperature and the like
Works in normal office environment where there is little discomfort to due to the extremes of heat, dust, noise, temperature and the like
