Clinical Social Worker is primarily assigned to a Day Rehabilitation or Outpatient setting. Clinical Social Worker utilizes individual and group therapy skills to assist patients and their families in achieving and maintaining a maximum level of physical, social and emotional functioning. Advises family caregivers, counsel's patients, and help plan a smooth transition to the next level of care.

Principal Responsibilitiesn

+ Interviews patients and family members to obtain information about home, social environment, family relationships, health history, patient's functioning and treatment goals.

+ Explains patient's background, needs, and family dynamics to the team and participates in the planning of a comprehensive post-hospital care program for the patient.

+ Assists with the decision-making process regarding discharge planning with the patient and family.

+ Determines the most effective social work treatment modality and carries out therapy to facilitate the adjustments to disability and resolve psychosocial problems with patient and/or family.

+ Implements patient/family discharge plan including: identification of D/C destination, Nursing home placements, DORS/DOA/Private Attendant Care Services, Home Health referrals, Continuum referrals (SRAlab), School reintegration, and Outpatient Services.

+ Identifies eligibility and assists family in applying for entitlements. Investigates alternative sources of funding for uncovered services in the community.

+ Participates in professional organizations and activities; assists in the orientation of other health care professionals in understanding the services and purpose of social work.

+ Participates in academics through teaching, research and publication.

Reporting Relationshipsn

+ Reports directly to Clinical Manager at designated site of care.

Knowledge, Skills & Abilities Requiredn

+ Master's Degree in Social Work from an accredited university required.

+ Current Illinois designation as a Licensed Clinical Social Worker (LCSW) required.

+ Demonstrated understanding of the principles, practices, techniques and professional standards in the field of social work.

+ Knowledge of community resources, health care system and interdisciplinary functioning.

+ Interpersonal communication, advocacy and analytic skills required to work with patients and families, and various medical professionals within and outside of the Institute.

Working Conditionsn

+ Normal patient care environment with little or no exposure to dust or extreme temperature.
