Job Summary


Provides social work assistance and facilitates completion of tasks to meet the social, emotional, financial, and environmental needs of the patient/family. Coordinates discharge planning and community referral for patients. Serves as a member of the multidisciplinary treatment team.

Essential Duties and Responsibilities:

Completes the patient’s social history and psychosocial assessment, including assessment of discharge planning needs.

Identifies treatment/discharge goals with the patient/significant others.

Identifies care planning needs and requests appropriate consultations as early as possible in the hospital stay.

Address and discuss discharge planning on an ongoing basis, complete residential placements and housing needs as needed; coordinates contacts with appropriate outside services and agency referrals, and arranges any needed supplies, equipment or transportation.

Contacts any referring agency to inform of discharge as appropriate.

Documents services provided with specific outcome goals stated.

Investigates case situations of patient/family and social background pertinent to diagnosis and treatment and presents this information to members of the health care team.

Promotes and models open communication among the multidisciplinary treatment team and facilitates the "team" approach.

Attends team meetings, providing relevant input, and facilitates multidisciplinary reports from team members.

Confers with other unit staff to monitor progress toward goals and to monitor services provided.

Reviews patient progress daily and makes appropriate adjustments to discharge plan as necessary

Serves as link between patient, physician, significant others and community.

Acts as patient/family advocate

Participates in Performance Improvement as requested.

Works closely with case managers and Clinical Manager to achieve departmental objectives

Attends workshops, seminars, in-services, both in-house and outside, to improve and update skills.

Complies with departmental, as well as the Medical Center policies and procedures.

Keeps case records current.

Keeps statistics current and compiled as directed.

Provides in-service training in the Medical Center and assists with community education programs.

Writes letters and correspondences as indicated.

Abides by the Professional Code of Ethics of Ohio Counselor and Social Work Board.

Licensed as a Social Worker in the State of Ohio

Bachelor’s degree required
