The VA Palo Alto Health Care System (VAPAHCS) consists of three inpatient facilities located at Palo Alto, Menlo Park, and Livermore, plus seven outpatient clinics in San Jose, Fremont, Capitola, Monterey, Stockton, Modesto, and Sonora. These facilities provide some of the world's finest medical care and cutting-edge technology.

VAPAHCS is a teaching hospital with an affiliation with the Stanford University School of Medicine which provides a rich academic environment including medical training for physicians in virtually all specialties and subspecialties. Our Health Care System operates over 800 beds, including three nursing homes and a 100-bed homeless domiciliary - all to serve more than 67,000 enrolled Veterans.

Work Schedule: Full-TimeMonday-Friday, 8:00am-4:30pm THIS POSITION IS TEMPORARY NOT TO EXCEED 2 YEARS AND MAY BE EXTENDED

VA Palo Alto Health Care system (VAPAHCS) is a large and complex VA health care system that provides veteran health care throughout a geographically large catchments area and is a major tertiary referral center. The Domiciliary Service at VAPAHCS provides supportive outpatient and/or residential rehabilitation to homeless and/or substance dependent Veterans with the ultimate goal of returning each individual to optimal independent living. In 1992 the Departments of Housing and Urban Development (HUD) and Veterans Affairs (VA) established the HUD/VA Supportive Housing (HUD/VASH) Program. The program is a collaborative effort, supported through HUD Section 8 rental assistance vouchers and VA's provision of intensive case management services. The primary goal of HUD/VASH is to move veterans and their families out of chronic homelessness. The primary component of the program is VA case management services, designed to improve veteran's health and mental health, enhancing their ability to remain stable, housed and community-integrated. The incumbent assists with program oversight, monitoring and administration, serving as a contact with local Public Housing Agencies (PHAs) and providing case management-based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA medical center, community health or mental health providers, and agencies that serve homeless populations.

The major duties and responsibilities include, but are not limited to:

  • The incumbent works closely with the VA Health Care System's (HCS) homeless programs, residential and community care programs, and domiciliary programs to ensure that coordination between these entities is facilitated and veterans direct care, referral, and follow up is contiguous and based on a continuum of services.
  • The incumbent independently provides clinical psychosocial and case management services at an advanced practice level for eligible veterans and their family members in the program.
  • The HUD/VASH Program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and federal agencies.

Qualifications

Basic Requirements: * U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. * Education. Must Possess a master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.[Transcripts must be submitted at time of application. Conferral of degree(s) must be stated on transcript(s). Unofficial transcripts(s) are acceptable]. * Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Exception. VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. This exception only applies up to the full performance level. For grade levels above the full performance level, the candidate must be licensed or certified. For grades below or at the full performance level, VHA social workers who are not licensed or certified at the time of appointment must become licensed or certified at the independent, master's level within 3 years of their appointment as a social worker. Most states require 2 years of post-MSW experience as a pre-requisite to taking the licensure/certification exam, and VHA gives social workers one additional year to pass the licensure/certification exam. In California, where the prerequisites for licensure exceed two years, social workers must become licensed at the independent, master's level within one year of meeting the full California prerequisites for licensure. Preferred Experience: At least one year outpatient mental health experience providing Housing oversight, working with Public Housing Agencies and providing case management services.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Grade Determinations: GS-9: Is the entry level grade for the GS-185 social work series and is used for social workers with less than one year of post-MSW experience and for social workers who are not yet licensed or certified at the independent practice level. Social workers at the GS-9 level are working toward completion of prerequisites for licensure or certification. In addition, the candidates must demonstrate the Knowledge, Skills and Abilities (KSAs) listed below: Ability to provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds.This requires knowledge of human development and behavior (physical and psychological), and the differential influences of the environment, society, and culture. Ability to work with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group, and family counseling skills.Work with more complex problems is done under close supervision.With guidance from the social work supervisor, ability to assess the psychosocial functioning and needs of patients and their family members, and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills, and assistance needed. Basic knowledge of psychosocial treatment modalities and, under supervision, ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals.This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. Ability to establish and maintain effective working relationships with clients, staff, and representatives of community agencies.Ability to communicate effectively, both orally and in writing, with people from varied backgrounds. Knowledge of medical and mental health diagnoses, disabilities, and treatment procedures.This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology.
