Responsibilities Req ID: 40221 Provides the expertise in areas relating to social service Demonstrates skills and knowledge to meet the physical, psychological aspect of all age groups Evaluates patient and family information, selects appropriate social work methods, and develops, implements and evaluates a plan in conjunction with the interdisciplinary efforts of the care team Actively participates and provides pertinent information in discharge planning meetings Provides support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations and bereavement Consults, assesses, refers, coordinates interventions in cases of suspected or actual situations of abuse, neglect or domestic violence Serves as the lead in obtaining legal guardianship and competency determinations Completes and disseminates all necessary legal and clinical documentation as required Serves as the lead in managing adoption related situations Works with patients and family members to listen to concerns and diffuse difficult situations Works with department managers to resolve patient concerns Works with physicians to resolve patient concerns Appropriately documents and communicates all patient concerns and compliments Utilize tools and resources within Allina in order to efficiently document and organize patient and family concerns Involves appropriate managers/staff/physicians in resolving concerns Discharge planning Professional accountability Communication Education Masters in Social Work preferred Bachelor Social Work degree accredited by the council on Social Work Education required Qualification/Skills Experience Minimum of one year experience in acute care preferred Additional training in acute, long term care and home health Previous experience in staff and community education preferred Skills Good communication skills Knowledge of adult learning principles Basic computer skills preferred Licensure/Certifications Certified or eligible for Wisconsin Social Work certification
