Position Focus: Provide clinical social work services at Yale Health. Provide patient assessment, evaluation and treatment planning; individual, group and couples therapy, crisis intervention and participate in after-hours on call rotation. Provide consultation to residential colleges.

Essential Duties 1. Completes psychosocial assessments, mental status examinations, and diagnostic assessments of both individuals and families and translates this information into medically necessary, multidisciplinary treatment plans. 2. Provides individual and group therapy consistent with evidence-based practice protocols for primary mental health, addiction, and co-occurring populations. Communicates with family, other disciplines and community agencies, as needed to ensure collaboration in treatment. 3. Provides clinical case management services, including referrals to appropriate levels of care. 4. Provides crisis intervention and clinical triage to facilitate necessary services, such as inpatient hospitalization, when indicated. 5. Determines membership and leads and facilitates psychotherapy groups. 6. Assists in determining appropriate circumstances for reintegration of patient into home and/or community. 7. Evaluates treatment progress and presents findings and recommendations at treatment team meetings. 8. Provides vocational, transitional, social adjustment, community living, and support services to patients. 9. Completes high quality, timely charting according to agency policies and procedures and payer requirements. 10. May perform other duties as assigned.
Required Education and Experience Master’s Degree in Social Work and one year of experience as a social worker in a psychiatric care setting or the equivalent combination of experience and education.
Required Skill/Ability 1: Comprehensive knowledge of outpatient assessment, diagnosis and psychotherapy.
Required Skill/Ability 2: Demonstrated ability in working in an outpatient mental health clinic with a wide variety of diagnostic groups.
Required Skill/Ability 3: Demonstrated ability in individual and group therapy and knowledge of early adult development.
Required Skill/Ability 4: Well-developed problem resolution, counseling and crisis intervention skills. Proven ability with emergency after-hours call.
Required Skill/Ability 5: Well-developed planning, oral and written communication and interviewing skills.
Preferred Education, Experience and Skills: Familiarity with Yale Health Mental Health & Counseling policies; experience working in a university mental health service; knowledge of Yale University and the residential college system.
Required Licenses or Certifications Licensure as a Social Worker (LCSW) in the State of Connecticut.
