Under the general direction of the Medical Director, the Palliative Care LCSW is responsible for providing clinical social work services to seriously ill patients and their families. You will work within a multidisciplinary team (physicians, nurse practitioners,, nurses and social workers) and coordinate with multiple community-based program including our outpatient clinic, inpatient services. home health, hospice, and nursing facilities.

Responsibilities:

Consult and collaborate with medical providers and other team members regarding the diagnosis and treatment of palliative issues. Provides palliative consultation to seriously ill patients and families, including comprehensive social work assessment, facilitation of goals of care and advance care planning discussions, facilitation of family meetings and assistance with transitional care plans.

Works closely with interdisciplinary team members, physicians, and unit team in care planning and provision.

Develop individualized care plans for patients based on needs assessments which identify problems, goals, skills, training and other service needs.

Provides short-term individual and family counseling.

Provides palliative care services in a rapid access model to patients with chronic health conditions that would benefit from behavioral or lifestyle change.

The ability and willingness to travel and to do home visits is required for this position.

The ideal candidate will have:

Current Maine State Board of Social Work Licensed Clinical Social Worker (LCSW) required

Advanced Certification in Hospice and Palliative Care Social Work (ACHPSWP) preferred or expected within two years of hire.

Up to 1 year or more: dedicated prior work experience with palliative care patient population preferred
