We are seeking a Social Worker to join the Case Management team at Johnston-Willis Hospital.

Case Management at Johnston-Willis Hospital is comprised of a team of skilled and compassionate Social Workers (SW) and Registered Nurses (RN). Our multi-disciplinary team serves as liaison and advocate for and between our patients, their families, the community, and the entire care team.

We cover all areas of the hospital including the Neonatal Intensive Care Unit (NICU), the Emergency Department, Post-Anesthesia Care Unit (PACU), as well as Same-Day Surgery. Using strong critical thinking skills our Social Workers and RNs multi-task in a fast-paced and ever-changing environment to develop, implement, and evaluate individualized patient care plans.

Our team collaborates closely with our community partners to provide services to our patients as needed such as home health, hospice, skilled nursing, assisted living, private care, durable medical equipment (DME), such as walkers, oxygen, etc. We also provide resources for those of our patients who are less fortunate and who may not have these resources. Our team environment supports professional development; several of our Social Workers are licensed LCSWs and several of our Registered Nurses are certified case managers through ANCC.

The Social Worker at Johnston-Willis Hospital: 1. Provides initial and ongoing psychosocial assessments for patients/families having complex psychosocial needs and/or medical complex diagnosis.

  1. Collaborates with the health care team and develops treatment goals and plans intervention that assist the patient and family to resolve psychosocial concerns that may arise as a result of illnesses, engagement in high risk social behaviors (i.e. substance abuse), limited finances and/or physical disabilities.

  2. Provides crisis and supportive counseling to enhance the patient’s and family’s problem solving and coping skills. Counsels patient/families regarding advance medical directives.

  3. Educates patients and families to the patient’s needs post discharge and informs them of the alternatives available to meet those needs.

  4. Refers patients who require extended care post discharge to long term care facilities. Once an appropriate bed is located, arranges for the patient’s transfer.

  5. Collaborates with community agencies to provide services to assist with housing, financial, transportation, psychosocial, educational, home health, and home medical equipment needs of patients/families post discharge.

  6. Documents social work assessments, treatment goals, interventions, outcomes, and discharge plans in the patient’s medical record congruent with departmental policies.

  7. Provides ongoing education for the other members of the health care team related to ethical, psychosocial and discharge planning issues.

  8. Maintains current knowledge of trends related to medical social work health care issues through attendance and active participation in continuing education and professional organizations.

    EDUCATION:

    • Required: Bachelor degree in Social Work or a bachelor degree and the equivalent in Social Work experience, knowledge and ability.

    • Preferred: Master of Social Work degree from an accredited school of Social Work. EXPERIENCE :

    • Required: Experience in discharge planning or case management.

    • Preferred: Two years of experience in a hospital or health care setting.
