Job Description:

This position assists patients and families who are requiring care in the adult Intensive Care Unit at University of Minnesota Medical Center. Primary duties include completion of a bio-psychosocial assessment, supporting patients and families through a variety of clinical treatment modalities with adjustment to episodes of acute or chronic illness, navigating staff and patients and families through complex legal and ethical issues related to hospitalization, crisis intervention, goals of care conversations, coordination and participation in both provider and patient/family care conferences, and addressing end-of-life issues when appropriate. The social worker collaborates with the Interdisciplinary Team to assist with progression of the hospital care plan as well as initiate discharge planning and transitioning of patients to the next level of care in the community. Knowledge of community resources (home care, nursing home and hospice) county, state and federal financial programs, chemical and mental health issues, criteria and procedure for civil commitments, child abuse, vulnerable adults, preferred.

Job Qualifications:


Master's degree in Social Work

Current LGSW



Previous experience in health care
